WHAT IS MOHS

Skin cancer is a prevalent concern, but the good news is that advanced surgical techniques have revolutionized the way we approach its treatment. Mohs Micrographic Surgery, or Mohs surgery, is a highly precise and effective method for removing skin cancer while minimizing damage to healthy tissue. Mohs Closures refer to the meticulous and specialized suturing techniques used to close surgical wounds following Mohs micrographic surgery. Mohs surgery is renowned for its ability to spare healthy tissue while excising cancerous cells layer by layer. The process involves the following steps:

  • Tissue Excision: The visible tumor is removed with a thin layer of surrounding tissue.
  • Mapping and Analysis: The removed tissue is meticulously mapped and examined under a microscope to identify cancerous cells. This process continues layer by layer until no cancer cells are detected.
  • Closure of the Wound: Once the cancer is entirely removed, the wound is ready for closure. This is a critical phase that requires careful consideration and skill to achieve optimal results, often the patient is sent to Dr. Voge for closure of the wound.

Proper closure techniques are essential for a swift and smooth healing process after skin cancer surgery, reducing the risk of complications, minimizing scarring and preserving your natural appearance. The closure phase involves suturing the wound left after the removal of cancerous tissue. Depending on the size and location of the wound, reconstruction may be necessary to restore the natural appearance and function of the treated area. This can involve techniques such as simple closure, skin grafts, or local flaps.

MOHS CLOSURE TECHNIQUES.

  1. Primary Closure: Ideal for Small Wounds: When the wound is small, it may be closed directly with stitches. This technique minimizes scarring and promotes quicker healing.
  2. Secondary Intention Healing: For Larger Wounds: In cases where closing the wound directly may cause tension or distortion, the wound is allowed to heal naturally over time. This method is suitable for larger defects and encourages tissue regeneration.
  3. Flaps: For Moderate-Sized Wounds: Flap techniques involve repositioning adjacent healthy tissue to cover the wound. This is particularly useful for larger defects, providing better cosmetic and functional outcomes.
  4. Grafts: For Extensive Defects: Skin grafts involve taking skin from one area of the body (donor site) and transplanting it to cover the Mohs surgery wound. This technique is employed for larger defects where direct closure or flaps may not be suitable. Is Mohs Closure Covered by Insurance?

Mohs Closure is typically covered by health insurance, as it is a necessary part of skin cancer treatment. However, it's essential to check with your insurance provider for specific details regarding coverage.

Should I bring someone with me?

It is often helpful to bring someone with you on the day of surgery. Depending on the size and location of your tumor, driving is sometimes a challenge after surgery, and having a driver with you is a good idea. Additionally, if someone other than the patient will be performing post-op wound care, the nurse can give this person direct instructions, demonstrate wound care, and answer any questions about the surgery.

What are the options for reconstruction after Mohs surgery?

The options for reconstruction depend on the anatomic location, depth, and extent of the Mohs defect. Reconstruction can be as simple as wound healing and allowing the area to close on its own.

Sometimes the defects can be closed in a straight line. Other times a skin graft is used (from near the ear or clavicle) or the local tissue is rearranged. Dr. Voge will attempt to hide the scars along existing facial creases and natural landmarks for the most aesthetic result.

When is reconstruction done after Mohs surgery?

Reconstruction is most commonly performed the same day or the day following the Mohs procedure.
