Understanding and Treating Melasma: Your Path to Even, Radiant Skin at Heartland Aesthetica
Melasma is one of the most common—and frustrating—skin concerns we see at Heartland Aesthetica. Those stubborn brown or grayish-brown patches that appear symmetrically across the cheeks, forehead, upper lip, nose, or chin can feel like they’re dimming your natural glow. While melasma itself is harmless, it often affects how people feel about their appearance, especially in sunny Kansas where UV exposure is a daily reality.
What Causes Melasma?
Melasma develops when melanocytes (the cells that produce pigment) become overactive. The main triggers include:
- Sun exposure — UV and visible light are the strongest drivers. Even brief or incidental exposure can darken existing patches or create new ones.
- Hormonal changes — Pregnancy (often called the “mask of pregnancy”), oral contraceptives, and hormone therapy frequently play a role.
- Genetics — A family history increases risk.
- Other factors — Certain medications, heat, and irritation from skincare products can contribute.
It is more common in women and in people with medium to deeper skin tones, but it can affect anyone. Melasma is chronic. It can improve significantly with the right approach, yet it tends to return if triggers aren’t managed carefully.
A Thoughtful, Multi-Layered Approach to Treatment
At Heartland Aesthetica, we do not treat melasma with a single product or procedure. Successful management usually requires a combination of consistent sun protection, targeted topicals, and carefully chosen in-office treatments tailored to your skin type, the depth of the pigment, and your lifestyle.
Daily broad-spectrum sunscreen is non-negotiable. We recommend a high-quality mineral or hybrid formula with SPF 30 or higher that also protects against visible light (tinted options containing iron oxides are especially helpful). Reapply throughout the day and pair it with hats and shade whenever possible.
Prescription and medical-grade topicals form the foundation for many patients. These may include hydroquinone (to reduce melanin production), tretinoin (to increase cell turnover and improve pigment clearance), and mild corticosteroids when inflammation is present. Newer non-hydroquinone brightening agents are also valuable for long-term maintenance and for patients who need to cycle off hydroquinone.
Chemical peels offer controlled exfoliation that helps lift pigmented cells. Glycolic acid and trichloroacetic acid (TCA) peels are commonly used to improve tone and texture while supporting clearer skin over a series of treatments.
Light and laser therapies, including BroadBand Light (BBL) and other Sciton technologies available in our practice, can target excess melanin. These treatments require expertise—settings must be conservative to avoid worsening pigmentation, particularly in darker skin types or active melasma. When performed thoughtfully, they can meaningfully even skin tone.

We also discuss hormonal contributors when relevant and may coordinate with your other healthcare providers. Cosmetic camouflage with high-quality makeup or tinted sunscreen provides immediate confidence while treatment progresses.
Setting Realistic Expectations
Melasma responds best to consistency and patience. Most patients see gradual improvement over weeks to months rather than overnight transformation. Maintenance is essential—once clearer skin is achieved, ongoing sunscreen use and periodic maintenance treatments help keep results.
Every treatment plan at Heartland Aesthetica begins with a thorough consultation. Dr. Vanessa Voge and our experienced team evaluate your skin, discuss your goals and medical history, and create a personalized strategy that prioritizes both safety and results.
